3.	Original Passport with valid Visa.
4.	Two passport sized coloured photographs in case the passport has not been issued by CGI Birmingham.
5.	Fee £20.00
6.	For Foreign Passport Holders : Naturalisation/ Nationality documents required
7.	Cancelled Indian Passport first & last page required (if in possession with the applicant)
