- As per instructions in general guidelines, printout of duly filled online application form available at https://mportal.passportindia.gov.in/mission/, should be submitted at VFS Centre.
- All particulars in the application should tally with those in the current passport. If there is a change in personal particulars, the requisite supporting document must accompany the application.
- Old passport will be retained during the period of the service.
Documents Required:
- Duly completed online application form (https://mportal.passportindia.gov.in/mission/)
- Current Passport in original along with self-attested copies of first (personal particulars), last (address) pages and any other endorsement pages and visa page of the passport and additional booklets, if any.
- Self-attested copy of UK Entry visa.
- Self attested copy of UK eVisa (online immigration status) along with share code or Indefinite Leave to Remain (ILR).
- Annexure E.
- Undertaking where Passport is withheld/ kept with UK Home Office.
- In case of change in address: Self-attested copy of address-proof (Election Commission ID card, water, electricity or telephone bill, nationalised bank passbook with 6 months bank statement).
- The passbook must have the permanent address mentioned and be attested/signed by a bank official along with the applicant's photograph.
- Redacted ID card issued by UIDAI, Government of India is also accepted.
- Documents must be in the applicant’s name. For minors, parent's passport copy will suffice.
Documents Required in Marriage, Divorce and Separated Case:
- For addition of spouse name: MARRIAGE CERTIFICATE or Joint Photo Declaration signed by both husband and wife (Annexure J).
- For deletion of Spouse Name: Divorce Order/Decree.
- For Change of Spouse Name:
1. Divorce Order/Decree or Death Certificate of first spouse AND
2. Re-marriage certificate or Joint Photo Declaration (Annexure J).
- For a change of surname by women (Marriage/Divorce):
- Marriage Certificate or Joint Photo Declaration (Annexure J).
- Divorce Order/Decree (if change is based on divorce).
- Full change of name: In case of a full change of name after marriage, the standard name change procedure should be followed.
Please Note:- Applicants are required to furnish original documents along with one set of self-attested photocopies of the same at the time of submission of application.
