Documents to be attached:

1.	Duly filled in Application form. Click here (to be filled in by the person accompanying the dead body)
2.	Death certificate from the relevant Local Authority.
3.	Cremation certificate.
4.	Certificate from the undertaker confirming that the urn contains ashes of the deceased person.
5.	Valid passport of applicant taking the ashes.
6.	Fee- £15
